The header parser uses forward and backward parsing, making the
bulletproof prevention of loops difficult, thus this simple
detection code.
If someone improves the forward/backward parsing so it cannot loop
then this commit should be reverted

The code that copies any extradata from the MXFDescriptor to the codec does

If no metadata follows MXF essence, that is essence ends with EOF,
"Operation not permitted" error was produced and a non-zero code
was returned from ffmpeg executable.

Correct bit mask for month/day/hour/min/sec values.
For reference the timestamp format specified in S377M is as follow:
year (int16), month (uint8), day (uint8), hour (uint8), sec (uint8),
msec (uint8).
A value of 0 for every fields means timestamp "unknown".

There's a a potential DoS problem in this function. Say an MXF file is
created with a PixelLayout with a long run of non-zeroes. Such a file could be
sent quickly (packed) over the net and would unpack quite fast. mxfdec would
then read it byte-by-byte, which would take considerable time.

Coverity thinks ofs can end up 15, thus writing past the end of layout[]. This
is incorrect since it's always incremented by 2. Checking ofs <= 14 makes
Coverity happy and doesn't hurt.

Fixes some DNXHD files generated by AVID TM, where codec UL was set to A-law
meanwhile the real audio codec was PCM S16. According to SMPTE RP 224, A-law is
the default value for sound essence parameters therefore we should handle it
specially.

The properties of the CDCI Descriptor are insufficient to specify
the pixel format for uncompressed picture data. SMPTE 377-1 and
RP224v10 have defined a set of picture coding labels to indicate what
formatting was used.
This patch uses 2 labels to detect UYVY422 or YUYV422 pixel formats.
It defaults to UYVY422 for 8-bit 4:2:2 pictures to support files
that were created before the coding labels were introduced ~2008
The codec pix_fmt default was changed from 0 (PIX_FMT_YUV420P) to
-1 (PIX_FMT_NONE)

probably meant to emulate av_log2_ceil(sizeof(..)) in cases where ".."
is not a power of two.
The reason for this is that such files have IndexTableSegments which when parsed
cover EditUnit ranges like this:
[0,1)
[249,250)
[249,377)
[0,249)
where each interval is [IndexStartPosition,IndexStartPosition+IndexDuration).
This would be reduced to a sparse index like:
[0,1), [249,250)
instead of the full range:
[0,249), [249,377)

This uses the old demuxing code for OP1a and separate demuxing code for OPAtom.
Timestamp output is added to the old demuxing code.
The seeking code is made to seek to the start of the desired EditUnit only,
from which the normal demuxing code takes over (if OP1a). This means we
do not use delta entries or slices, only StreamOffsets. OPAtom seeking
basically works like before.
This also makes D-10 seeking behave the same way as OP1a and OPAtom. In other
words, we allow seeking before the start or past the end for D-10 too.

Specifically, this means parsing as before until we run into essence.
At that point we seek to the footer and parse until EOF. After that we start
seeking backward to the previous partition and parse that until we run into
essence or the next partition. This procedure is repeated until we encounter
the last partition we parsed in the forward direction.
The end result of all this is that large essence containers are not needlessly
parsed. This speeds up parsing large files a lot.

This patch also computes the size of the packets when unknown.

These values include KAGSize, HeaderByteCount and IndexByteCount.
The length of the pack itself is also stored, and KAGSize is sanity checked.
The FATE sample has KAGSize == 0, which is adjusted to 512.
Other bad KAGSizes are set to 1.
Specifically, this means parsing as before until we run into essence.
At that point we seek to the footer and parse until EOF. After that we start
seeking backward to the previous partition and parse that until we run into
essence or the next partition. This procedure is repeated until we encounter
the last partition we parsed in the forward direction.
The end result of all this is that large essence containers aren't needlessly
parsed. This speeds up parsing large files a lot.

Manual replacements are done in this commit.
In many cases, the id is some constant made up number (e.g. 0 for video
and 1 for audio), which is then not used in the demuxer for anything.
Those ids are removed.

This also lists the objects from those two libraries as internal (by adding
the ff_ prefix) so that they can then be hidden via linker scripts.

prototype and also always call them with exactly those arguments.
The previous way seems to have worked on all supported platforms, however
it was not strictly valid C and would crash e.g. with a compiler that
uses stdcall by default.
Also avoids warnings if -Wstrict-prototypes is used.
