c740f737e9 vorbisdec: Use int instead of uint16_fast_t for index variables 
uint16_fast_t is unsigned int (or long) on Linux, which when compared
with int results in an unsigned compare.

37c506e8b9 stream_selection can be freed in the fail case, in which case it's unassigned. 
Therefore, init it with NULL to prevent a crash on invalid streams.

0a24503d04 Remove CCITT fax G4 tests (partial revert of r24954). This test is 
corrupting memory somehow and segfaulting in the BSDs.
