89da5781a1 Call av_find_stream_info() immediately after opening an input file. 
Needed to avoid reading the same TS two times at the beginning of the
file (whic resulted in an "error, non monotone timestamps..." when
streaming RTP from a file)

750f0e1f8b Introduce two new logging functions av_hex_dump_log() and av_pkt_dump_log() 
which use av_log() for logging instead of fprintf().
