Revert "all: Don't set AVClass.item_name to its default value"
...
Some callers assume that item_name is always set, so this may be
considered an API break.

lavfi: add a flag for filters able to work with hw_device_ctx
...
This way the caller can set it just on the filters that can make use of
it.
