999c23a13a Make avfilter_insert_filter() print the instance name of the 
auto-inserted scale filter, rather than the filter name, as this
provides more information.

1b168a428d Ignore 0-height slice in sws_scale, otherwise a final 0-height slice will 
cause libswscale to assume the incorrect slice order and complain about
slices starting in the middle.
